Need help caring for your disabled child?

Caring Hearts Agency, Inc. provides services, in direct correlation with the Missouri Healthy Children and Youth (HCY) Program, in the privacy and comfort of your own home. The Healthy Children and Youth (HCY) Program is a comprehensive, primary and preventive health care program for recipients of MO HealthNet and includes service coordination and authorization for medically necessary services for children and youth from birth to age 21 with special health care needs. Service coordination includes assessment through home visits and links to services and resources. If your child has special health care needs, Caring Hearts Agency, Inc. can enable them to remain safely in their homes with their families.

Our Children and Youth Services

Service Coordination for the HCY Program includes:

Evaluation and assessment of needs | Identifying and accessing service providers | Service plan development and implementation | Coordination of services through resource identification and referral | Family support | Assisting in establishing a medical home | Transition planning

 

Prior authorization of medically necessary services including:

Private duty nursing | Personal care needs | Skilled nursing visits | Authorized Registered Nurse (RN) visits | Administrative case management | In-home personal care | n-home nursing care

 

Caring Hearts Agency, Inc. can provide services for children with the following qualifications:

Be a Missouri resident | Be birth to age 21 | Need medically necessary services | Be enrolled in MO HealthNet
